Job summary

Solera Senior Living is seeking a Wellness Nurse (LPN) at Echo Lake to deliver high-quality care for residents. You’ll administer medications, support daily wellness routines, supervise care staff, and prioritize residents’ health and dignity in a team-focused environment.

The role requires an active LPN license in the state and at least 1 year of nursing experience, with Alzheimer’s/dementia experience preferred.

Qualifications

• Active LPN license in the state of employment.
• Minimum 1 year of nursing experience; Alzheimer's/dementia experience preferred.
• Strong communication, interpersonal, and leadership skills.
• Compassionate, patient-centered approach to care.
• Strong observational skills and attention to detail.
• Annual flu vaccination required (medical/religious exemptions accepted).

Responsibilities

• Provide direct nursing care and supervise medication care specialists.
• Administer medications, treatments, and procedures per physician’s orders.
• Document resident care and update care plans as needed.
• Monitor for changes in residents’ health and communicate updates to physicians and families.
• Assist with resident move-ins and orientation.
• Provide leadership and coaching to care staff, including evaluations.
• Participate in emergency response procedures and uphold safety standards.
• Maintain accurate resident records and ensure compliance with policies and state regulations.
• Promote resident independence, wellness, and participation in activities.
• Foster a positive, respectful environment for residents, families, and team members.

Wellness Nurse (LPN) Role Overview:

As a Wellness Nurse (LPN), you’ll play a vital role in delivering high-quality care to our residents. You will administer medications, support daily wellness routines, supervise care staff, and help ensure each resident’s health and dignity are prioritized.
