Job summary

Trulee Evanston in Evanston, IL is seeking an experienced Wellness Nurse (LPN) to deliver high-quality care to residents and support daily wellness routines. You will administer medications, document care plans, supervise care staff, and communicate with physicians and families to ensure health and dignity.

Qualifications

• Active LPN license in Illinois.
• Minimum 1 year of nursing experience; Alzheimer’s/dementia experience preferred.
• Strong communication, interpersonal, and leadership skills.

• Strong observational skills and attention to detail.
• Annual flu vaccination required (medical/religious exemptions accepted).

Responsibilities

• Provide direct nursing care and supervise care staff.

• Monitor changes in health and communicate updates to physicians and families.

Wellness Nurse (LPN) Role Overview

As a Wellness Nurse (LPN), you’ll play a vital role in delivering high-quality care to our residents. You will administer medications, support daily wellness routines, supervise care staff, and help ensure each resident’s health and dignity are prioritized.

• Wellness Nurse Key Responsibilities:
  • Provide direct nursing care and supervise medication care specialists.
  • Administer medications, treatments, and procedures per physician’s orders.
  • Document resident care and update care plans as needed.
  • Monitor for changes in residents’ health and communicate updates to physicians and families.
  • Assist with resident move-ins and orientation.
  • Provide leadership and guidance to care staff, including performance evaluations and coaching.
  • Participate in emergency response procedures and ensure safety standards are upheld.
  • Maintain accurate resident records and ensure compliance with community policies and state regulations.
  • Promote resident independence, wellness, and participation in daily activities.
  • Foster a positive, respectful environment for residents, families, and team members.

Wellness Nurse Qualifications
