Job summary

Solera Senior Living is seeking a Wellness Nurse (LPN) to deliver high-quality care to our residents and supervise care staff. You will administer medications, support daily wellness routines, and help ensure health and dignity are prioritized within our vibrant community.

We value compassion, teamwork, and strong leadership as you contribute to residents’ wellbeing. This PRN role covers Sundays and Mondays 11am–7pm, with opportunities for growth and comprehensive benefits.

Qualifications

• Active LPN license in the state of employment.
• Minimum 1 year of nursing experience; Alzheimer’s/dementia experience preferred.
• Strong communication, interpersonal, and leadership skills.
• Compassionate, patient-centered approach to care.
• Annual flu vaccination required (medical/religious exemptions accepted).

Responsibilities

• Provide direct nursing care and supervise medication care specialists.
• Administer medications, treatments, and procedures per physician orders.
• Document resident care and update care plans as needed.
• Monitor for changes in residents’ health and communicate updates to physicians and families.
• Assist with resident move-ins and orientation.
• Provide leadership and guidance to care staff, including performance evaluations and coaching.
• Participate in emergency response procedures and ensure safety standards are upheld.
• Maintain accurate resident records and ensure compliance with community policies and state regulations.
• Promote resident independence, wellness, and participation in daily activities.
• Foster a positive, respectful environment for residents, families, and team members.

Wellness Nurse (LPN) Role Overview:
