Job summary

Sunshine Retirement Living seeks a Wellness Nurse in Texas to provide person-centered care under LPN or RN scope. Qualifications

• Current, valid LPN or RN license is required.
• Experience in long-term care, acute care, or geriatric nursing preferred.
• Supervisory experience preferred.
• Ability to read, write, and speak English professionally with staff, families, and external customers.
• No disqualifying criminal offenses per regulatory guidelines.
• Ability to read, analyze, and interpret general materials and regulations.
• Ability to work under time constraints and meet deadlines.
• Maintain a positive, respectful team approach and be a team player.
• Ability to write nursing/patient progress notes and nursing procedures.
• Knowledge of dementia care practices or willingness to complete memory care training.
• TB clearance and regulatory background checks where applicable.

Responsibilities

• Provides resident care in accordance with accepted standards of practice within the LPN or RN license scope.
• Routinely observes and reports on residents’ conditions and documents them.
• Administers medication and treatments as prescribed within license scope.
• Receives, transcribes, and executes physician orders.
• Implements and evaluates resident care plans.
• Directs caregivers to ensure care is provided per standards and policies.
• Coaches care staff in managing memory loss with interventions and redirection.
• Ensures proper documentation in electronic health records.
• Performs other duties as assigned.

The overall purpose of the Wellness Nurse position is to provide Person-Centered Care consistent with accepted standardsandexpectationsofSunshineRetirementLivingandstateregulations. TheWellnessNurse'sresponsibilities include, but are not limited to, monitoring resident health status, providing physical care, and early diagnosis and intervention of resident changes in health status. Wellness Nurses must communicate politely and effectively with family, primary physicians, and perform all administrative charting or other services as required by regulations and company policy. In assisted living and memory care, the Wellness Nurse ensures care practices that promote safety, dignity, and engagement for residents with cognitive impairment, while serving as a clinical resource and role model for caregiving staff.
