Coterie Senior Living - Cathedral Hill in San Francisco, CA seeks a Wellness Nurse to support the Wellness Manager in providing resident care and coordinating medications. This role emphasizes compassionate patient care, adherence to training, and collaboration with a diverse care team.

Responsibilities include supervising medication activities, aiding residents with daily living, and contributing to assessments and care plans.

Qualifications

• Licensed Vocational Nurse/Licensed Practical Nurse or Registered Nurse with a current license in good standing.
• First Aid and CPR certification if required by State.
• Must complete Atria's Medication Management Training if providing medication reminders or assistance with medication.

Responsibilities

• Assist in supervising the medication program and medication room activities.
• Assist residents with Activities of Daily Living, as needed.
• Maintain positive relations with residents, families, and physicians.
• Report significant incidents to the General Manager/Wellness Manager.
• Assist in completing resident assessments and service plans.
• Help train Assisted Living staff and participate in orientation of new staff.
• Attend scheduled in-service programs, training, and staff meetings.
• Support scheduling and staffing of Wellness Assistants and Medication Assistants as needed.
• Perform other duties as assigned.

Qualifications

Responsibilities

The Wellness Nurse is responsible for assisting the Wellness Manager with the overall implementation, delivery, and coordination of Resident Care services at the Community. The Wellness Nurse will also assist in monitoring the medication management program.

• Assist in supervising the medication program, activity in the medication room, and assisting the residents with self-administration of their medications.
• Assist residents with Activities of Daily Living, as needed.
• Maintain positive relations with residents, families, and physicians.
• Report any significant incidents and/or changes in the resident's needs to the General Manager/Wellness Manager.
• Assist in the completion of resident assessments and services plans.
• Assist in the training of Assisted Living staff and participates in orientation of new staff.
• Attend and participates in scheduled in-service programs, training programs, and staff meetings.
• Assist with scheduling and staffing of Wellness Assistants and Medication Assistants as needed.
• May perform other duties as needed and/or assigned.

Salary Range

The wage range for this position is $36.90/hr - $44.28/hr, dependent on prior work history and experience
