• Georgia LPN/LVN or RN license in good standing is required.
• Experience in assisted living, memory care, senior living, or related healthcare setting preferred.
• Strong leadership, communication, assessment and documentation skills.
• Ability to supervise, train and support nursing and resident care team members.
• Familiarity with electronic medication administration systems, including QuickMAR, is preferred.

Responsabilidades

• Team Leadership and Staffing: Ensure daily Resident Assistant assignment sheets are completed and followed.
• Team Leadership and Staffing: Provide hands-on training and orientation for CMAs, medication technicians, nurses, and resident care team members.
• Team Leadership and Staffing: Supervise team members to ensure care follows policies and resident care plans.
• Team Leadership and Staffing: Reassign team members for call-outs, scheduling changes, or census changes.
• Team Leadership and Staffing: Support nurses and medication technicians with clinical questions and needs.
• Team Leadership and Staffing: Provide meal and break coverage for the day-shift nurse as needed.
• Clinical Oversight: Participate in daily clinical stand-up meetings.
• Clinical Oversight: Assess residents with condition changes and document/communicate with families and physicians.
• Clinical Oversight: Review daily 24-hour log and follow up on changes and charting.
• Clinical Oversight: Review HOT Box and ensure follow-up is completed.
• Clinical Oversight: Conduct daily rounds and provide clinical guidance across the community.
• Clinical Oversight: Support Evergreen Director with clinical oversight and resident care needs.
• Clinical Oversight: Organize clinical information for visiting physicians and nurse practitioners.
• Medication and Documentation Management: Review and approve medication orders in QuickMAR.
• Medication and Documentation Management: Ensure monthly weights and vital signs are recorded.
• Medication and Documentation Management: Complete quarterly self-administration assessments for residents on self-medication.
• Medication and Documentation Management: Review new resident records for accuracy and completeness.
• Medication and Documentation Management: Complete monthly medication cycle check-ins for Assisted Living, Bridges, and Evergreen.
• Medication and Documentation Management: Review medication discrepancy reports and resolve concerns.

Qualifications

• Current Georgia LPN/LVN or RN license in good standing

• Previous clinical experience in assisted living, memory care, senior living, or a related healthcare setting preferred

• Strong leadership, communication, assessment, and documentation skills

• Ability to effectively supervise, train, and support nursing and resident care team members

• Familiarity with electronic medication administration systems, including QuickMAR, is preferred

Key Responsibilities

Team Leadership and Staffing

• Ensure daily Resident Assistant assignment sheets are completed and followed

• Provide hands‑on training and orientation for newly hired CMAs, medication technicians, nurses, and resident care team members

• Supervise team members to ensure care is delivered according to company policies and each resident’s individualized service plan

• Reassign team members as needed to address call‑outs, scheduling changes, or changes in resident census

• Support nurses and medication technicians with clinical questions and resident care needs

• Provide meal and break coverage for the day‑shift nurse as needed

Clinical Oversight

• Participate in daily clinical stand‑up meetings

• Assess residents experiencing changes in condition and ensure appropriate documentation and communication with families and physicians

• Review the daily 24‑hour communication log and follow up on resident changes and required charting

• Review the HOT Box and ensure all necessary clinical follow‑up is completed

• Conduct daily rounds and provide clinical guidance throughout the community

• Support the Evergreen Director with clinical oversight and resident care needs

• Organize relevant clinical information for visiting physicians and nurse practitioners

Medication and Documentation Management

• Review and approve medication orders in QuickMAR

• Ensure monthly resident weights and vital signs are completed and entered accurately

• Complete quarterly self‑administration assessments for residents who manage their own medications

• Review new resident records for accuracy and completeness using the required checklist

• Complete monthly medication cycle check‑ins for Assisted Living, Bridges, and Evergreen

• Review medication discrepancy reports and ensure concerns are resolved promptly
