Job summary

Spectrum Retirement Communities is seeking a Wellness Nurse to support residents through clinical monitoring and coordination with the care team. You will assist with admissions, ensure care aligns with plans, and support physicians and nursing staff in maintaining high regulatory standards.

The role requires an active nursing license (LPN/RN), experience in geriatric assessment, and a collaborative, compassionate approach to resident care. Full-time daytime/evening shifts available.

Qualifications

• Three years' experience in assessment and geriatric nursing (preferred).
• Current nursing license and state registration (LPN or RN).
• Working knowledge of state regulations, policies, and procedures.

Responsibilities

• Coordinate resident admission process including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Administer medications and treatments per physicians' orders.
• Coordinate care and services with outside providers including resident physicians.
• Document and report care provided and resident observations including changes.
• Maintain knowledge of state regulations, policies, and procedures.

Job Description

Schedule: Full Time Days 6am-2pm, Evenings 2pm-10pm

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.

• Assist in coordination of the resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Medication and treatment administration and ensure medications are administered per physicians' orders
• Assistance with coordinating care and services with other outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain a working knowledge of state regulations, policies, and procedures

Qualifications

• Working knowledge of state regulations, policies, and procedures, dictated for compliance
• Graduate from an accredited school of practical nursing
• Current nursing license and state registration (LPN or RN)
• Three years' experience in assessment and geriatric nursing, preferred
