About Acenda Integrated Health

Acenda Integrated Health provides 100+ behavioral health programs, including mental health services, crisis care, substance use recovery, family therapy, maternal and pediatric support, and residential programs across New Jersey.

Program Overview

We are committed to enriching individuals’ lives by promoting a supportive environment. The goal of our residential program is to provide skill teaching, education, and encouragement of personal growth, fostering independence in activities of daily living so residents can integrate into the community. Our setting is a semi‑supervised apartment that aims to be the least restrictive yet clinically appropriate structured setting, promoting physical and mental health and reducing the need for psychiatric hospitalization.

Job Overview

Our Residential Service Program offers a continuum of housing for the acutely and chronically mentally ill with supervised residences. As a Care Manager in our Adult Residential Program, you will provide safety and security for adult clients within a semi‑supervised apartment setting and promote a healthful, supportive environment. You will work closely with the residential nurse, supervisor, and director to implement individualized rehabilitation treatment goals, using evidence‑based practice.

Key Responsibilities

• Establish and maintain positive relationships with residents to foster a supportive and caring environment.
• Provide assistance and guidance in daily living activities, including personal care, meal planning, and household chores.
• Implement and monitor individualized treatment plans, ensuring residents’ physical, emotional, and social well‑being.
• Facilitate group activities and therapeutic programs that promote skill development, socialization, and community integration.
• Maintain accurate and timely documentation of residents’ progress, incidents, and significant observations.
• Collaborate effectively with a multidisciplinary team, including residential supervisor, nurse, director, and medical professionals.

Qualifications

• Minimum: High school diploma or equivalent, plus two years of related mental health experience with professionalism and a commitment to excellence in care.
• Must have and maintain a valid driver’s license with an acceptable driving record.
• Preferred: Baccalaureate degree from an accredited college or university with a major in mental health, health, or another related human services discipline, and experience with the chronic mentally ill adult population.
• Preferred: Previous case management experience.

Employment Details

Starting Base Rate: $20 per hour.

Schedule: 20–29 hours per week, evenings and weekends required.

Bilingual rate: an additional $1.50 added to the base rate for Spanish proficiency.
