Qualifications

  • Bachelor’s Degree with 2 years’ experience in the human services, psychology or related field.
  • Must have a valid driver’s license and insured vehicle.
  • Strong interpersonal and communication skills."

Responsibilities

  • Serve as contact point for clients, care team, family/caregivers and community resources regarding treatment plans.
  • Monitor adherence and progress, facilitate changes as needed.

Work to support clients to obtain treatment services and maintain care to achieve wellness. Work with a team to ensure clients needing help to achieve mental wellness are wrapped with all needs of service. Educate clients regarding their health care condition and provide follow-up with clients and providers to ensure client needs are met. Operate as a strong community liaison to ensure that individuals can navigate and understand all services that are available to them for their mental health needs. Collaborate with Acenda and Community Providers to link individuals into services needed.As an integrated health organization Acenda values the use of evidence-based practice or EBP's.$700 Sign on Bonus Is AvailableKey Responsibilities:Serve as the contact point, advocate and informational resource for clients, care team, family/caregivers and community resources regarding client's treatment planPartner with clients to monitor adherence to their treatment plan, evaluate effectiveness, monitor progress in a timely manner and facilitate changes as required.Facilitate client linkages, referrals and access to appropriate treatment, community supports and medical or specialty providers according to the treatment planEducate patient and family/caregiver(s) about relevant community resources and provide resources/connection to IFSS.Conduct clinical follow up when clients are receiving inpatient or emergency care, and ensure a seamless transition back into the treatment plan upon discharge.Cultivate and maintain relationships with external community providers and supports.Completes and submits accurate reports and chart documentation in a complete and timely manner according to program guidelines and established procedures.Ensures quality and accuracy of written and verbal communications.Compile data for and prepare a variety of reports as required.
