Job summary

Community Bridges, Inc. (CBI) is seeking a Wellness Navigator to help remove barriers to health care access.

You will work with medical teams and community partners to proactively manage patient care and support patients, families, and caregivers in connecting to prevention, diagnosis, treatment and follow-up services. The role emphasizes in-home and community-based care coordination, education, advocacy, and system navigation to improve outcomes for individuals experiencing crisis, SUD, and

Qualifications

• High school diploma or GED required.
• Associates degree or higher in behavioral health preferred.
• Education and/or experience in health and nutrition preferred.
• Arizona Driver’s License required.
• Clean 39-month driving record (MVR).
• Behavioral Health Technician (BHT) certification required per policy.

Responsibilities

• Provide case management, education, care coordination, and advocacy.
• Coordinate care with medical providers and community agencies.
• Assist patients and families to access prevention, diagnosis, timely treatment, and follow-up.
• Work primarily in-home and in community settings to remove barriers to care.

Job Summary

The Wellness Navigator will help to remove personal and environmental obstacles to health care access. The Wellness Navigator is responsible for working closely with the primary medical provider, the clinical team, and staff of other agencies involved in the patients care to proactively manage patient care by providing case management, education, care coordination, systems navigation, advocacy and support primarily in-home and/or in the community. The Wellness Navigator will engage, educate and offer support to individuals, their family patients, and caregivers to connect them to prevention, diagnosis, timely treatment, and follow-up.

Skills/Requirements:

• Highschool diploma or GED required. Associates degree (or higher) in a field related to behavioral health preferred.
• Education and/or experience in the field of health and nutrition preferred.
• Or equivalent combination of education and experience.
• Current AZ Driver’s License.
• Clean 39-month MVR.
• Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures required.
• Arizona Fingerprint Clearance card required.
