Mosaicms in Oregon City is seeking a compassionate Wellness Manager with an LPN license to lead resident care in our community. You will supervise care teams, review plans, and ensure compliant medication administration.

This full-time on-site role emphasizes collaboration, regulatory knowledge, and hands-on leadership to support residents and families while maintaining high clinical standards. Experience with EMRs is preferred.

Competenze

Requires high school diploma or equivalent.
Active LPN/LVN license in good standing per state regulations.
Minimum 2 years supervisory experience in senior living, assisted living, or healthcare preferred.

Mansioni

Clinical Oversight & Resident Care: oversee delivery of resident care, assessments, service plans, and progress documentation.
Participate in move-in evaluations and ongoing assessments per Mosaic policies and state regulations.
Review and document resident updates daily in the community log; attend service plan reviews.
Review physician orders, MARs, and care documentation for accuracy and compliance.
Ensure medication administration and care procedures align with regulatory and clinical best practices.
Escalate unresolved clinical issues or medication discrepancies to RN or Executive Director as needed.
Oversee the medication delivery system to ensure safety, accuracy, and compliance.
