Job summary

The Bungalows at Springdale is seeking a Wellness Nurse (LPN) to join the team. The LPN reports to the Wellness Director and works PRN with days and every other weekend required. Responsibilities include medication administration, resident care, and PSP documentation.

Requirements include LPN or RN licensure and at least 1 year of experience with elders or disabled individuals, with leadership capabilities. Weekend/evening flexibility is needed in this role.

Qualifications

• Must be a Licensed Practical Nurse or Registered Nurse.
• Minimum of one year of experience working with elders or disabled individuals preferred.
• Demonstrated leadership capabilities.
• Experience in Assisted Living or Dementia care preferred.
• Ability to work weekends, evenings and flexible hours.

Responsibilities

• Monitor health and wellbeing of residents and provide leadership support in resident care and services.
• Administer medications and communicate with resident support services and families.
• Maintain clinical quality assurance and reflect wellness program components.
• Complete and document monthly wellness visits and Personalized Service Plans (PSP).
• Inform Wellness/Executive Directors of changes in medication or service levels.
