Heritage Senior Living is seeking a compassionate Wellness Coordinator to lead the resident care team in Monona, WI. You will coordinate daily care, monitor ISPs, and support CNA/Resident Assistants to maintain high-quality, individualized services.

Responsibilities include directing rounds, managing staffing, documenting vitals and wellness data, and collaborating with the DON and families. This role blends hands‑on care with leadership to ensure exceptional resident experience.

Qualifications

  • High school diploma or GED required.
  • CNA certification preferred but not required.
  • Experience in senior living or caregiving role is strongly preferred.

Make Every Day Better.Heritage Senior Living is seeking a compassionate, organized, and dependable Wellness Coordinator to help lead our resident care team and ensure residents receive consistent, high-quality, individualized care.Reporting to the Director of Nursing (DON) or designee, the Wellness Coordinator provides hands-on leadership and support to CNA/Resident Assistants while coordinating daily resident care needs, wellness activities, staffing, documentation, and communication.This is an ideal opportunity for an experienced caregiver, Resident Assistant, CNA, or care team leader who is ready to take on greater responsibility and make an impact on both resident care and team performance.What You’ll DoCoordinate Daily Resident CareComplete morning and afternoon rounds throughout assigned areas.Review shift updates and the 24-hour communication report to maintain continuity of care.Speak with care team members at the beginning of the shift to identify resident needs, changes, and priorities.Follow up throughout the day to ensure resident needs and assigned care responsibilities are completed.Read and understand resident Individualized Service Plans (ISPs) and help ensure services are provided according to each resident's plan.Contribute observations regarding resident needs, preferences, and changes to the service-planning process.Complete scheduled wellness checks and respond promptly to resident needs.Promptly communicate significant changes or concerns to the LPN, DON, or other appropriate clinical team member.Lead & Support the Care TeamProvide day-to-day leadership and direction to CNA/Resident Assistants.Confirm assigned employees are present and appropriately deployed within their assigned areas.Coordinate and adjust assignments and staffing based on resident and community needs.Train, mentor, coach, and support CNA/Resident Assistants.Participate in employee coaching and corrective-action processes as appropriate.Assist with new Resident Assistant orientation and required skills checks.Monitor employee documentation and assignment completion.Help ensure employees follow resident ISPs, safety procedures, infection-control practices, and community standards.Lead by example and provide hands-on resident care when needed.Coordinate Vitals, Weights & Wellness MonitoringCoordinate and delegate assigned resident weights and vital signs to appropriately trained CNA/Resident Assistants.Follow up to ensure assigned weights and vital signs are completed and documented accurately.Monitor completion of required vital signs associated with antibiotics, falls, or other identified resident needs.Communicate abnormal findings or concerns promptly to the appropriate licensed clinical team member.Help ensure clinical follow-up responsibilities are completed as directed.Coordinate Showers, Baths & LaundryMaintain and coordinate resident shower and bathing schedules according to ISPs and resident preferences.Follow up with employees to ensure scheduled showers and baths are completed and documented.Help coordinate resident laundry responsibilities and follow up on completion.Address missed or incomplete services promptly to help ensure residents receive consistent care.Support Hospice & Outside ProvidersMaintain current information regarding residents receiving hospice services.Coordinate hospice schedules, including nursing/aide visits and showers provided by hospice personnel.Provide appropriate resident information, such as weights and bowel-movement documentation, to hospice providers as authorized and required.Follow up regarding durable medical equipment (DME) provided through hospice, including wheelchairs, cushions, beds, and mechanical lifts.Communicate with hospice and other outside providers to support coordinated resident care.Support Diet & Nutrition CommunicationHelp ensure resident ISPs accurately reflect current diet instructions.Communicate diet changes promptly to culinary staff and appropriate care team members.Follow up on identified dietary needs and concerns.Communicate resident changes affecting diet or nutrition to the appropriate licensed clinical team member.Support Life Safety & Quality AssuranceAssist with routine quality and life-safety monitoring.Complete or coordinate checks related to:Elopement risks and applicable safety measures.Durable medical equipment.Resident pendants and call systems.Other assigned quality or safety measures.Identify concerns and ensure appropriate follow-up.Maintain emergency binders and help ensure required information is current, complete, and accurate.Follow established resident-safety, infection-control, and emergency procedures.Lead Daily Care FocusLead daily Care Focus meetings when on duty.Coordinate participation from the appropriate clinical and care team members.Ensure responsibility for Care Focus is appropriately communicated to PM and NOC supervisors when applicable.Meet regularly with the DON to identify Care Focus topics and plan flash training.Gather employee feedback regarding resident changes, supplies, staffing, and other care needs.Escalate urgent resident or operational concerns to the LPN and/or DON promptly.Help ensure important information is communicated across shifts.Communicate With Residents, Families & the Care TeamMaintain consistent communication among residents, families, caregivers, CNA/Resident Assistants, LPNs, the DON, and other departments.Participate in weekly Clinical Care Coordination meetings.Communicate effectively with physicians, pharmacies, hospice providers, and other outside providers as appropriate.Build positive relationships with residents and families.Respond to resident and family questions or concerns professionally and ensure clinical concerns are directed to the appropriate licensed team member.Provide Hands-On Resident SupportAssist residents with activities of daily living according to their ISPs and applicable training.Support residents with grooming, bathing, oral hygiene, dressing, eating, toileting, incontinence care, mobility, transfers, and other identified needs.Respond promptly and courteously to resident pages and alarms.Assist residents with appointments and community programs.Use proper body mechanics and approved resident-assistance equipment.Promote resident independence, comfort, privacy, dignity, choice, and individuality.Observe residents for changes in condition or behavior and promptly report concerns.Perform other
