Job summary

Carey Services, Inc. is seeking a Wellness Coordinator to ensure coordination of health services for individuals served. You will interact directly with individuals, train staff, consult with providers, and keep the Interdisciplinary Team updated on health matters.

This role requires an LPN or RN license, a valid driver’s license, and the ability to learn basic teaching principles. Strong communication, teamwork, and time-management skills are essential.

Qualifications

• Must possess a Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
• Valid driver’s license and insurability requirements.
• Ability to learn basic teaching principles as instructed.
• Good math computation and English writing ability.

Responsibilities

• Coordinate health services for individuals receiving wellness coordination.
• Maintain up-to-date medical records for all individuals served.
• Educate and train staff and individuals on health and safety matters.
• Implement agency policies related to contagious diseases.
• Participate in IPP and team meetings to address medical needs.
• Coordinate wellness initiatives for eligible individuals.
• Monitor state and federal regulations to maintain compliance.
• Support administration in maintaining health & safety policies.
• Promote agency-wide safety and preventive maintenance.
