Job summary

The City of Chicopee’s Council on Aging Department is hiring a Wellness Health Nurse to provide on-site consultations to older adults and frail homebound seniors. The nurse will manage records, follow-ups, and coordinate with physicians and families to ensure comprehensive care.

Responsibilities include initial assessments, monitoring health indicators, medication guidance, nutrition counseling, and arranging supplies for the Wellness Center.

Qualifications

• Registered Nurse Master’s Degree is required.
• Four or more years of related experience and/or training in elder care or equivalent.
• Current license to practice as Registered Professional Nurse in Massachusetts is required.

Responsibilities

• Perform initial assessments for residents aged 60+ in Chicopee.
• Follow up, monitor, and refer as necessary; counsel clients and families on healthcare.
• Measure health indicators (blood pressure, glucose, anemia, weight, depression) and provide related care.
• Instruct on medication management and equipment use; administer vitamin B-12 shots as prescribed.
• Maintain accurate health records for each client and order supplies for the Wellness Center.
• Provide in-home nursing services when needed and support health promotion programs.

Salary Range: $28.73/Hour - $37.61/Hour

Schedule: Monday - Friday

Certifications/Licenses Required: Bachelor's Degree In Nursing, Current license to practice as Registered Professional Nurse in Commonwealth of Massachusetts.

SUMMARY

The City of Chicopee's Council on Aging Department is hiring a Wellness Health Nurse who will provide on one consultation with older adults and/or their families and/or their physician/pharmacist. The Nurse also provides the same services to frail homebound older adults. Responsible for all record keeping and follow-ups.

ESSENTIAL DUTIES INCLUDE

• Provides initial assessments to Chicopee residents who are sixty (60) years or older.
• Follow-up, monitors and makes referrals, as necessary. Counsels clients and/or family regarding health care and problems.
• Tests blood sugar levels, high blood pressure, anemia, weight loss or gain and/or depression, as necessary.
• Informs older adults on proper medication management, medication information, instruction, instruction on use of medical equipment, give Vitamin B-12 shots as prescribed by a physician, does dressing changes and does assessment for unhealthy use of alcohol and/or prescription drugs. Provide nutrition counseling based upon evidence-based programs.
• Keeps accurate health records on each client.
• Orders all supplies necessary for operation of the Wellness Center
• Performs physical exams.
• Informs the Social Services Coordinator and/or other staff as appropriate of any changes in the elder's well-being or follow up that might be necessary.
• Advocates or informs physicians and/or family and/or other health services of any follow-up care which the client might need.
• Orders necessary supplies.
• Facilities various support groups, including a support group for diabetics.
• Plans for health promotional programs.
• Provides in-home nursing services for those who have no alternative.
• Performs other duties as required.

QUALIFICATIONS

• Registered Nurse Master's Degree (M.A) – Required
• Or four or more years related experience and/or training of direct care nursing elders; or equivalent combination of education and experience.
• Current license to practice as Registered Professional Nurse in Commonwealth of Massachusetts. – Required
