Job summary

StoryPoint and Independence Village is seeking a Wellness Director to direct clinical resident care services, uphold 1440 care standards, and lead a team to deliver high-quality wellness initiatives.

You will oversee evaluations, care conferences and service plans, coordinate with pharmacy, and maintain compliance while building strong partnerships with community leaders.

Flexible scheduling including on-call, weekends, and holidays may be required as part of maintaining outstanding resident care.

Qualifications

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) licensed in the state is required.
• CDP certification required within 90 days of hire.
• Assisted Living/Memory Care experience preferred.
• Leadership experience is preferred.
• Proven ability to manage revenue and financials successfully.

Responsibilities

• Lead, manage and hold team accountable for performance.
• Oversee revenue for Wellness, Evaluations, Care Conferences and Service Plans.
• Coordinate Alzheimer’s and Dementia Program activities.
• Understand and lead to 1440 care standards.
• Communicate vision to align organizational, community and wellness goals.
• Forge partnerships with community leaders and external partners.
