Job summary

You will oversee service plans, care coordination, and collaboration with physicians and staff to ensure the highest quality of life for residents while supporting family communications and staff development.

Qualifications

• Active and unencumbered LPN license required.
• Licensed Practical Nurse credential required to supervise nursing team

Responsibilities

• Make a significant contribution to the quality of life of residents by maintaining high levels of care and consistency.
• Review routines and calendars; coordinate resident appointments, treatments, or procedures
• Assist with new resident orientation and transition to the community
• Develop, implement and update service plans
• Make daily rounds to observe and evaluate residents’ physical and emotional status
• Coordinate admission and discharge of residents
• Evaluate staff performance and manage recruiting, hiring and training
• Ensure documentation of resident clinical status and communication with physicians and other healthcare professionals
• Ensure medications and supplies are ordered, in stock and counted at shift change
• Demonstrate organizational time-management skills
• Promote Cascade Living’s mission, values and beliefs

As the Wellness Director (Health Services) at Mt. Bachelor Assisted Living and Memory Care, you’re the clinical leader and oversee the nursing team within the Memory Care department. It’s your responsibility to guide and encourage staff and collaborate with other leaders in the community to meet the comprehensive needs of our residents.

What we look for in a Wellness Director:

• Maintains a current state license as a professional Licensed Practical Nurse LPN
• Compassionate individuals who will represent our community in a positive and professional manner
• Effective communicator who connects with staff, residents and their families
• Demonstrate active listening techniques, gain support by treating others with dignity and respect
• Strong customer service skills with ability to problem solve
• Work with the dining room manager to identify dietary needs and develop appropriate dietary plans
• Ownership attitude who is able to stay focused despite interruptions throughout the day
• Ability to work flexible hours to include evenings and weekend

$80,000 - $95,000/annually
