Job summary

Dominion Care is seeking Wellness Coaches to support youth in foster care, high-risk pregnancies, and mental health needs. Coaches will coordinate outreach, connect individuals with services, and help achieve personal health and wellness goals.

We offer competitive pay of $18.00–$22.00 base, flexible scheduling, and a range of benefits including medical/dental, PTO, and retirement options. Training will be provided to ensure effective care coordination across the Virginia community.

Qualifications

  • Bachelor's degree in social work, psychology, nursing, counseling, or related behavioral health field.
  • Minimum 6 months of outreach/case management experience (preferred).
  • QMHP registration (preferred).
  • Valid Virginia Driver’s license, proof of current insurance and willingness to use personal vehicle.

Responsibilities

  • Coordinate intensive outreach and case management services to members and ensure continuity of care.
  • Assist members in Crisis and Safety planning.
  • Coordinate with medical and mental health providers to enhance the care network.
  • Advocate for members to ensure delivery of services and accompany during appointments.
  • Maintain records related to assessments and service plans.

Our Wellness Coaches are responsible for member outreach for those in need of increased support and services as the result of foster care transition, high-risk pregnancy, or mental health conditions.
