Job summary

The Ponce Care Center in St. Augustine, FL is seeking a Weekend Wound Care Nurse to join our dedicated team. You will perform wound assessments, develop care plans, educate residents and families, and supervise CNAs to ensure high-quality care for our residents.

We require a Florida-licensed registered nurse with at least one year in a long-term care setting, plus strong leadership and teamwork abilities. This on-site role includes participation in an on-call rotation and a supportive work

Qualifications

• Must hold a current Florida nursing license in good standing.
• Minimum of one year of experience in a long-term care setting.
• Must be a supportive team member and demonstrate teamwork.
• Must possess leadership and supervisory ability and work harmoniously with staff.

Responsibilities

• Perform wound assessments, reassessments, and management for all residents.
• Create plan of care with goals, treatment, evaluation and outcomes.
• Educate residents, families, and staff on wound management and therapies.

Essential Duties & Responsibilities of the Wound Care Nurse:

• Perform wound assessments, reassessment, and management for all residents.

• Create plan of care including goals, treatment, evaluation and outcomes of residents.

• Provide education to residents, family, and staff regarding wound management, prevention, support, and other therapies.

• Direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs and other nursing personnel.

• Demonstrate knowledge and skill to develop and implement policies and procedures to manage residents with wounds.

Included in the On Call rotation

Job Requirements of Wound Care Nurse:

• Must be a Licensed Nurse in good standing and currently licensed by the state where our facility is located (FL)

• Minimum of one year of experience in long-term care setting

• Must be a supportive team member, contribute to, and be an example of teamwork.

• Must possess leadership and supervisory ability and willingness to work harmoniously with staff.
