Job summary

The Ponce Therapy Care Center in St. Augustine, FL is seeking a Weekend Wound Care Nurse to join our team. This role focuses on wound assessment, plan of care, and education for residents, families, and staff, while supervising day-to-day nursing activities.

Qualified candidates hold an RN license in Florida with at least one year in long-term care and demonstrated leadership abilities to contribute to a high-quality, supportive care environment for residents and colleagues.

Qualifications

• Must be a Licensed Nurse in good standing and currently licensed by the state (FL).
• Minimum of one year of experience in long-term care setting.
• Must possess leadership and supervisory ability and willingness to work harmoniously with staff.

Responsibilities

• Perform wound assessments, reassessment, and management for all residents.
• Create plan of care including goals, treatment, evaluation and outcomes.
• Provide education to residents, family, and staff regarding wound management and therapies.
• Direct nursing care to residents and supervise day-to-day nursing activities by CNAs and other staff.

Essential Duties & Responsibilities of the Wound Care Nurse:

• Perform wound assessments, reassessment, and management for all residents.
• Create plan of care including goals, treatment, evaluation and outcomes of residents.
• Provide education to residents, family, and staff regarding wound management, prevention, support, and other therapies.
• Direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs and other nursing personnel.
• Demonstrate knowledge and skill to develop and implement policies and procedures to manage residents with wounds.

Included in the On Call rotation

Job Requirements of Wound Care Nurse:

• Must be a Licensed Nurse in good standing and currently licensed by the state where our facility is located (FL)
• Minimum of one year of experience in long-term care setting
