Job summary

Jade affiliated facility in Chicago is seeking a part-time RN or LPN Wound Care Coordinator for the weekend shift. You will deliver quality wound care and preventive measures to maintain skin integrity for residents in compliance with federal and state guidelines and facility policies.

Qualifications

• Active Illinois nursing license.
• Current CPR certification.
• At least two years of RN/LPN experience.
• Ability to lift at least 75 pounds.
• Proficiency in English (spoken and written).
• Capable of handling work-related stress.

Responsibilities

• Assess and document residents' wound status and skin integrity.
• Plan, implement, and evaluate wound care per policy.
• Coordinate rounds and interdisciplinary care plan reviews.
• Take physician orders and ensure required documentation.
• Initiate nutritional interventions to support wound healing.
• Perform direct resident care and administer treatments as ordered.
• Conduct frequent rounds to ensure proper pericare and preventive ointments.
