Job summary

Jade Financial seeks a dedicated RN or LPN Wound Care Coordinator for weekend shifts at our Extended Care affiliated facility in Chicago. You will deliver high-quality wound care and preventive skin measures to residents in line with federal and state guidelines.

The role requires nursing licensure, CPR certification, and at least two years of RN/LPN experience. You will work weekends, with a comprehensive benefits package and weekend-oriented schedule to support patient recovery.

Qualifications

• RN or LPN with valid nursing license.
• Current CPR certification.
• At least two years of RN/LPN experience.
• Ability to lift and move at least 75 pounds.
• Must be able to speak and write English.
• Must cope with mental and emotional stress.

Responsibilities

• Assess and manage residents' wound and skin integrity using the nursing process.
• Document wound status and conduct rounds.
• Coordinate care plans and interdisciplinary communication.
• Administer treatments as ordered by physicians.
• Provide direct resident care and ensure timely peri-care and ointment application.
• Maintain safety and compliance with policies.

We are seeking a dedicated RN or LPN Wound Care Coordinator for the weekend shift. The Wound Care Nurse is responsible for delivering quality wound care and preventative measures to maintain skin integrity of assigned residents in compliance with applicable federal and state guidelines and established policies and procedures.

RN or LPN Wound Care Coordinator must have the following:

• Nursing License required by the state.
• Must have current CPR certification.
• Must have, as a minimum, two (2) years of experience as an LPN/RN.
• The physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job. Must be able to lift and move items and patients weighing at least 75 pounds.
• Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
• Must be able to speak and write the English language.
• Must be able to cope with the mental and emotional stress of the position.

As a skilled, compassionate RN or LPN Wound Care Coordinator, you will:

• Utilizes the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of residents wound and skin integrity, including rounds, measurements, interdisciplinary care plan review, treatment recommendations, physician orders, and required documentation.
• Collecting and documentation of residents’ skin status including medical history.
• Wound care prevention and prevention through plan of care.
• Sets priorities for nursing action for wound care and prevention.
• Communicates with physicians regarding resident wound needs.
• Initiates nutritional interventions to ensure that residents with wounds are provided nutrition to promote wound healing.
• Performs direct resident care utilizing established procedure, policies, and standards.
• Administer treatments and procedures in accordance with the physician’s orders.
• Conducts rounds frequently to ensure that residents with incontinence are receiving pericare and preventative ointments timely.
• Performs duties in a safe manner in accordance with established policies and procedures.
