Job summary

Monarch Communities is seeking an LPN Wellness Nurse to provide direct nursing care to residents with an emphasis on holistic wellness. You will supervise daily nursing activities by CNAs and work under the Health and Wellness Director to ensure high-quality care.

The role requires an active LPN license and prior experience in long-term care or related settings, with strong knowledge of regulations and excellent computer skills.

Qualifications

• Graduation from an accredited School of Nursing with a current LPN license.
• Minimum 2 years’ experience in long-term care, assisted living, home health or hospital setting preferred.
• Experience with residents with Alzheimer’s or other dementias.

Responsibilities

• Monitors the health, safety, and well‑being of all residents.
• Assists in training and monitoring of medication administration.
• Supervise care staff in accordance with current state regulations and community policies.
• Maintains clinical quality assurance per federal, state and local standards.
• Promote the highest degree of service to residents while exemplifying the company mission.
• Conduct thorough resident assessments.
