Job summary

Concierge Home Care is seeking an experienced Weekend Virtual Care Nurse (LPN) to join our remote team serving Florida residents. The role focuses on reducing hospitalizations and improving outcomes through proactive remote care, with a weekend schedule and a pay range of $20.00–$22 per hour.

Responsibilities include virtual visits, triage, chronic disease management, and collaboration with care teams, all while maintaining HIPAA-compliant documentation and telehealth workflow.

Qualifications

• Active, unencumbered Florida LPN license.
• Minimum of 2 years of home health experience.
• Experience with triage and hospitalization prevention.
• Strong clinical judgment and communication skills.
• Comfortable using telehealth platforms and EMR systems.
• Reliable internet connection and private remote workspace.

• Perform triage and identify early signs or risk trends.
• Support chronic disease management and patient education.
• Monitor telehealth data and elevate concerns appropriately.

Schedule & Compensation

• Days: Friday, Saturday & Sunday
• Hours: 8:00 AM – 8:00 PM
• Position Type: Remote / Virtual Care
• Pay Range: $20.00 – $22 per hour

What You’ll Do

Qualifications

Required:

• This is a 100% work-from-home position. Candidates MUST reside within the state of Florida due to operational, licensing, and/or business requirements.

• Strong clinical judgment and communication skills
• Comfortable using telehealth platforms and EMR systems
• Reliable internet connection and private remote workspace

Preferred:

• Experience working with high-risk or medically complex patients
• Knowledge of chronic disease management
