Job summary

Concierge Home Care in Port St. Lucie, FL is seeking a licensed LPN to deliver high-quality nursing care in patients' homes based on individualized care plans.

You will manage acute and chronic conditions, medication administration, and post-surgical rehab, while educating patients and caregivers to support recovery and independence. You’ll work with a supportive team, enjoy flexible scheduling (full-time, modified full-time, or PRN), and access ongoing training, mentorship, and benefits

Qualifications

• Unencumbered Florida LPN License (required).
• Valid driver’s license, auto insurance, and reliable transportation (required).
• IV Certification (required).

Your Role as a Licensed Practical Nurse (LPN):

• Deliver high-quality nursing care to patients in their homes based on individualized care plans.
• Perform a variety of tasks, including:

• Medication management and administration.
• Assessing patient conditions and reporting changes.
• Developing tailored, goal-oriented treatment plans.
• Post-surgical rehabilitation.
• Educating patients and caregivers to support recovery and independence.
• Pain management and wound care.

Qualifications:
