Job summary
Concierge Home Care in Florida is seeking a Weekend Virtual Care Nurse (LPN) to join our remote team for Fridays to Sundays, 8 AM to 8 PM. This 100% work-from-home role focuses on high-quality patient care and proactive remote management.
You will conduct virtual visits, triage, chronic disease support, and collaborate with care teams while maintaining HIPAA compliance. A Florida LPN license and 2 years of home health experience are required.
Qualifications
- Active Florida LPN license.
- Minimum of 2 years of home health experience.
- Experience with triage and hospitalization prevention.
- Comfortable using telehealth platforms and EMR systems.
Responsibilities
- Conduct virtual patient visits and clinical assessments.
- Perform triage and identify early warning signs or risk trends.
- Support chronic disease management and patient education.
- Monitor telehealth data and escalation concerns appropriately.
- Help prevent avoidable ER visits and rehospitalizations.
- Collaborate with interdisciplinary care teams.
- Document patient interactions accurately and timely.
- Participate in care planning and performance improvement initiatives.
- Support development of virtual care best practices.
