Job summary

Concierge Home Care is seeking a Home Health Licensed Practical Nurse (LPN) to provide skilled nursing care in the St. Augustine area.

The role offers a flexible schedule, autonomy, and 1:1 patient care in patients' homes, with coverage that minimizes drive time through clustered scheduling. You will assess patients, administer medications, educate families, and collaborate with a multidisciplinary team while using AI-assisted charting and WellSky/Kinnser EMR.

Qualifications

• Active Florida LPN license.
• Valid driver’s license and reliable transportation.
• CPR certification.
• Home health experience preferred, but not required.

Responsibilities

• Provide skilled nursing care under supervision of a Registered Nurse.
• Perform patient assessments and assist with care planning.
• Administer medications and treatments as ordered.
• Educate patients and caregivers on disease management and care plans.
• Monitor patient condition and report changes to the supervising RN.
• Collaborate with physicians and interdisciplinary teams.
• Complete documentation using AI-assisted tools.

Your Role as a Home Health LPN

• Provide skilled nursing care under the supervision of a Registered Nurse
