Job description

We are expanding and in need of Care Team Coordinators to work in the Western suburbs in and around New Lennox.

This full-time position is ideal for a Caregiver with at least 2 years of experience. Experience must be on the job and not care for own family.

The ideal candidate will have experience with personal care with both mobile and bed-bound clients.

The role involves being in the field supporting our clients as needed but ALSO includes working in the office to learn administrative skills and support the office team as needed.

Duties Include:

  • Coordinating caregivers, including support with hiring, training, performance improvement, and implementing caregiver training programs in collaboration with the supervisor
  • Conducting quality assurance site checks to ensure care plans are correctly implemented, policies followed, and job duties performed satisfactorily, meeting the Senior Helpers brand standards.
  • Reviewing daily logs and timesheets to ensure adherence to care plans and identifying lost hours.
  • Supporting with new hires, terminations, and inactive employees, communicating with the Scheduling Manager.
  • Covering caregiver call-offs daily or weekly, either by helping find replacements or going in person to cover shifts, ensuring clients are not left without care.
  • Collaborating with Human Resources regarding coaching and training of staff; ensuring accurate and thorough communication.
  • Adhering to federal, state, and local laws and regulations.

Qualification Required:

  • CNA certification is an advantage
  • Minimum of 2 years of experience as a Caregiver, including personal care, companionship, and support for clients with dementia
  • Excellent organizational skills, attention to detail, and ability to prioritize in a changing environment
  • Critical thinking skills to proactively prevent and solve problems
  • Ability to visit clients' homes in the assigned territory
  • Ability to learn software programs quickly
  • Strong communication skills
  • Ability to work independently and as part of a team
  • Excellent punctuality
