Job summary

Brookhaven Health Care Facility in East Patchogue, NY seeks an RN Unit Coordinator (Weekends Only) to oversee the clinical operations of a nursing unit, ensure high-quality resident care, regulatory compliance, and staff supervision on Saturdays and Sundays. Rate: $45-50/hour; Shift: Part-Time; Schedule: 7 am - 3 pm.

Qualifications

• Associate Degree in Nursing required; NYS RN license required.
• 1 year RN experience in long-term care required.
• 3-5 years long-term care supervisory experience preferred.

Responsibilities

• Oversee the clinical care and daily operations of an assigned nursing unit.
• Supervise nursing staff, nursing assistants, rehabilitation aides, and unit support staff.
• Conduct resident assessments and ensure care plans are implemented and followed.
• Monitor resident conditions and communicate changes with providers, families, and interdisciplinary team members.
• Review provider orders, lab results, incident reports, and quality indicators.
• Participate in care plan meetings, family meetings, QAPI activities, and interdisciplinary rounds.
• Coordinate admissions, discharges, transfers, and resident services.
• Ensure documentation, medication management, and regulatory compliance.
