Brookhaven Health Care Facility in East Patchogue, NY is seeking an RN Supervisor to lead nursing operations on assigned shifts and ensure residents receive high-quality care. You will support staff, ensure regulatory compliance, and promote a safe environment for residents and families.

Qualifications

• Associate Degree in Nursing required; Bachelor's Degree preferred.
• New York State RN License required.
• 1 year RN experience in geriatric or rehabilitation setting; 3+ years preferred.
• Prior leadership or supervisory experience preferred.
• Strong clinical assessment, critical thinking, and decision-making skills.
• Excellent communication, leadership, and conflict resolution abilities.
• Knowledge of long-term care regulations, infection control practices, and resident rights.
• Ability to lead teams and manage multiple priorities in a fast-paced environment.

Responsibilities

• Supervise nursing staff during assigned shifts (team leaders, CNAs, trainees).
• Ensure care plans are implemented, updated, and followed.
• Conduct resident assessments and monitor condition changes; communicate with providers/families.
• Oversee medication administration, treatments, documentation, and nursing services.
• Address concerns from residents, families, visitors, and staff to promote a positive care experience.
• Assist with staffing coverage, scheduling adjustments, and overtime authorization.
• Support staff education, orientation, coaching, and performance management.
• Ensure compliance with facility policies, infection control, resident rights, and regulatory requirements.
• Respond to emergencies and facility operational issues; participate in QAPI initiatives.
• Perform additional duties as assigned.

RESPONSIBILITIES:

• Support staff education, orientation, coaching, and performance management activities
• Ensure compliance with facility policies, infection control standards, resident rights, and regulatory requirements
• Respond to emergencies, safety concerns, incidents, and facility operational issues
• Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
• Perform additional duties as assigned

REQUIREMENTS:

• Associate Degree in Nursing required; Bachelor's Degree preferred
• Valid NYS Registered Nurse (RN) License required
• Minimum of 1 year of RN experience in a geriatric or rehabilitation setting; 3+ years preferred
• Prior leadership or supervisory experience preferred
