Job summary

Oak Hills Care Center is seeking an experienced nurse to provide skilled nursing care under the medical direction of the attending physician. You will oversee CNAs and licensed nurses and collaborate with the interdisciplinary team to assess, plan, implement, and evaluate individualized resident care plans.

The role requires clinical assessment, care coordination, medication administration, and adherence to facility policies with a focus on resident-centered care and safety.

Qualifications

• Requires licensed RN or LPN in Oklahoma.
• Minimum two years of nursing experience in long-term care or similar setting.
• Ability to lead CNAs and coordinate with IDT.
• Excellent communication with residents, families and staff.

Responsibilities

• Provide skilled nursing care under medical direction.
• Oversee CNAs and licensed nurses.
• Coordinate care plans with the IDT and physicians.
• Complete required MDS assessments and charting.
• Administer medications as ordered and monitor adverse effects.
• Participate in safety, quality and education activities.
