Concierge Home Care is seeking a Clinical Care Coordinator, RN to support our clinical team in Heathrow, FL. This is a Monday–Friday, primarily office-based role with local travel as needed and a strong focus on coordinating care and ensuring Medicare compliance.

You will guide and coach clinicians, review documentation for regulatory standards, and monitor patient outcomes while promoting a culture of quality and collaboration. Competitive salary plus quarterly bonuses and benefits are included.

Qualifications

• Active Florida RN license required.
• Minimum 2 years RN experience.
• At least 1 year skilled home health experience.
• Strong understanding of home health regulations and Medicare compliance.
• Excellent communication and leadership skills.

Responsibilities

• Support and coordinate day-to-day clinical operations of the home health team.
• Provide guidance and coaching to home health clinicians.
• Review clinical documentation for Medicare compliance.
• Monitor quality of care, patient outcomes, and clinical performance.
• Collaborate with physicians and interdisciplinary team members.
• Travel locally to support clinical operations.
