Plainfield Health Care Center is seeking dedicated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) for Weekend Option coverage. The role delivers high-quality nursing care in a skilled nursing facility, overseeing clinical care, medications, and assessments while coordinating with the interdisciplinary team.

Weekend shifts total 32 hours with premium pay per facility policy. Indiana licenses, strong clinical skills, and teamwork are essential for success in this role.

Qualifications

• RN or LPN with Indiana license in good standing.
• Graduate of an accredited nursing program.
• Experience in skilled nursing/long-term care preferred.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing setting.
• Complete comprehensive resident assessments and monitor changes.
• Administer medications and treatments as ordered.
• Document nursing care and outcomes in the EMR system.
• Supervise CNAs and other staff as assigned.
• Communicate changes in condition to physicians and team.
• Participate in care planning and interdisciplinary meetings.
• Respond to emergencies and resident needs promptly.
• Ensure infection control and CMS/state regulatory compliance.
• Promote resident rights and person-centered care.
• Foster teamwork and professional communication.
