BAYADA Home Health Care in York, PA is seeking a Per Diem Weekend Registered Nurse (RN) to join our home health senior living team. You will provide skilled nursing and rehabilitative care to clients in their homes, helping them stay independent and safe.

You will assess patients, manage acute and post-acute needs, perform wound care, medication reconciliation, and educate families. The role requires PA nursing licensure and at least one year of relevant experience.

Qualifikationen

• A current nursing license in good standing in the state of Pennsylvania.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings).

• Perform head-to-toe assessments and identify changes in patient condition in the home setting.
• Triage and manage urgent clinical situations independently in the home setting.
• Perform wound assessment, care, and documentation.
