Job summary

BAYADA Home Health Care is seeking a Per Diem Weekend Registered Nurse (RN) to join our home health senior living team. You will provide skilled nursing and rehabilitative care to clients in the home, supporting the goal of remaining in a familiar environment.

Qualified candidates will have a PA RN license, one year of relevant experience with adults/geriatric populations, and the ability to work independently while delivering compassionate, high-quality care across the senior living territory.

Qualifications

• RN license in good standing in Pennsylvania
• Minimum 1 year hands-on experience with adult/geriatric populations in acute care, ED, home health, hospice, ICU, med-surg, sub-acute or rehab settings

Responsibilities

• Perform head-to-toe assessments and identify changes in patient condition
• Triage and manage urgent clinical situations in the home setting
• Carry out medication reconciliation and patient teaching
• Conduct comprehensive wound assessment, care and documentation
• Utilize point-of-care documentation and technology to maintain accurate records
