Job summary

Qualifications

• Graduate from an accredited School of Nursing (ADN or higher; BSN preferred).
• Current RN license in the practicing state or active multi-state license.
• 1-3 years of clinical hospital nursing experience; 2 years in Case Management preferred.
• Case Management or Utilization Review certification preferred; InterQual experience preferred.

Responsibilities

• Coordinate the plan of care for patient stays, manage length of stay and discharge planning.
• Facilitate progression of care and identify patients at risk during transitions.
• Assess psychosocial, medical, and discharge needs and reassess as indicated.
• Ensure post-acute care needs are appropriate in partnership with Social Services.
• Evaluate progression using InterQual criteria and escalate as needed.
• Refer to third-party payer and disease management programs for chronic patients.
• Act as liaison between physicians, patients, families, hospital staff and outside agencies.
