Job summary

HCA Florida Memorial Hospital in Jacksonville, FL seeks a PRN Registered Nurse Case Manager to coordinate patient care over weekends. The RN will assess needs, plan safe discharges and collaborate with the care team to optimize outcomes.

Qualifications

• Requires active RN license in state of practice (FL).
• Experience in hospital-based case management or discharge planning is preferred.
• Knowledge of InterQual criteria and post-acute care processes preferred.

Responsibilities

• Coordinate plan of care and manage length of stay with safe discharge planning.
• Identify patients at risk during transition of care.
• Assess psychosocial, medical and discharge needs of patients/family, and reassess as indicated.
• Ensure post-acute medical needs and level of care in partnership with Social Services.
• Evaluate progression of care using InterQual criteria throughout the episode of care.
• Make referrals to third-party payers and disease/case management programs.
• Liaise with physicians, patients/family, hospital staff and outside agencies as an advocate.
