Job summary

Assisted Home Health & Hospice in Glendale, AZ is seeking a Weekend Warrior RN Case Manager for Fri/Sat/Sun coverage in the East Side of Arizona. This full-time role requires Oasis SOC experience and on-call availability.

Qualifications

• Must have experience with Oasis paperwork.
• Must be available for on-call (day/night).
• 4 Starts of Care (Oasis SOC) per day.

Responsibilities

• Complete initial and ongoing comprehensive assessments of the patient’s needs.
• Provide service and/or treatments requiring substantial and specialized nursing skills.
• Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs.
