Job summary

Amedisys is seeking a Registered Nurse Case Manager for full-time weekend coverage (Saturday and Sunday) plus one weekday. You will assess patients, coordinate care with the team, and support home health outcomes.

Must have home health and OASIS experience and be licensed in the applicable state(s). Earns $83,000–$90,000 annually with a comprehensive benefits package including medical plans, PTO, HSA options, wellness rewards, counseling services, and a 401(k) match.

Qualifications

• 1+ year of clinical RN experience.
• Current RN license for the state(s) assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and appropriate rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
