Job summary
Amedisys is seeking a Weekend RN Case Manager to support home health patients in a patient-focused environment. You will assess patients, coordinate with the care team, and help patients learn self-care and rehabilitation techniques.
The role requires RN licensure, CPR certification, and a valid driver's license. Weekend shifts are complemented by a weekday component and comprehensive benefits.
Qualifications
- 1+ year of clinical RN experience.
- Current RN license for applicable state(s).
- Current CPR certification.
- Valid driver’s license, reliable transportation and liability insurance.
Responsibilities
- Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
- Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
- Promotes patient health and independence through teaching and rehabilitative measures.
- Supervises LPNs and HHAs.
- Completes documentation timely and accurately.
- Regularly communicates patient progress to the clinical manager and care team.
- Plans and provides staff education.
- Performs on-call responsibilities and on-call services to patients/families.
- Participates in clinical development and continuing education programs.
- Other duties as assigned.
