Job summary

Amedisys Home Health is seeking an RN Case Manager to join our weekend team. The role involves patient assessments, collaborating with the care team, and developing plans of care to support home-based healing.

This position requires an active RN license, CPR, a valid driver's license, and at least 1 year of clinical experience. Shift is Saturday and Sunday 12-hour, with additional weekday hours as needed; compensation ranges from $75,000 to $85,800 with comprehensive benefits.

Qualifications

• RN with 1+ year of clinical experience; VP approval if less than 1 year.
• Current RN license for the assigned state.
• CPR certification is required.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to plan care.
• Refer to other disciplines as needed and document rationale if not referred.
• Promote health and independence through teaching and rehabilitative measures.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Communicate patient progress to the clinical manager and team.
• Plan and provide staff education.
• Perform on-call responsibilities for patients/families as assigned.
• Participate in ongoing clinical development and CE programs.
• Other duties as assigned.
