Job summary

Amedisys is seeking a Weekend RN Case Manager for Home Health in Tulsa, OK. This RN will assess patients, plan and coordinate care with the team, and promote home-based recovery in a supportive setting.

Shifts are 12 hours on Saturday and Sunday plus 10–12 hours on select weekdays. Competitive pay and comprehensive benefits accompany opportunities for growth within a leading home health and hospice organization.

Qualifications

• One or more years of clinical RN experience required.
• Current RN license for the assigned state.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Supervises LPNs and HHAs.
• Completes timely and accurate documentation.
• Regularly communicate patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
