Job summary
A leading home health agency is seeking a Part-time Registered Nurse Case Manager to conduct assessments and make home visits within designated territories. The ideal candidate has an active CA RN license and experience with patient care assessments. Qualifications
- Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired.
- High level of clinical knowledge in home health care is highly preferred.
- Experience with high acuity patients is a plus.
Responsibilities
- Conduct Start of Care (SOC) Admissions - OASIS.
- Make home visits to patients in designated territory.
- Monitor clients’ conditions, reporting changes to Clinical team.
