Job summary

Saint Francis Health System – Yale Campus in Tulsa, OK is hiring a Case Manager (RN) on the Prem Weekend schedule (Friday, Saturday and Sunday 7am–7pm). The role provides case management services, focusing on discharge planning and transitions, coordinating with the care team to develop safe discharge plans.

You will work with patients and families, ensure payer authorizations for post-acute services, and help reduce readmissions while navigating barriers to discharge.

Qualifications

• Has completed the basic professional curricula of a school of nursing and holds or is entitled to hold a diploma or degree.
• Must hold a valid RN license (multistate or Oklahoma). CCM preferred.

Responsibilities

• Plans patient care with physicians and the healthcare team.
• Ensures the care and discharge plans align with the clinical course and needs.
• Identifies potential barriers to discharge and coordinates planning.
• Participates in multidisciplinary patient rounds.
• Coordinates discharge planning with the health care team, patient and caregivers.
• Evaluates progress toward clinical/functional goals.
• Arranges post-acute care and payer authorization.
• Coordinates hospital-to-hospital transfers.
• Tracks avoidable days and reports findings to leadership.
