Saint Francis Hospital, Inc. in Tulsa, OK is seeking a Case Manager (RN) to provide discharge planning and care coordination for patients on the Prem Weekend shift (Fridays, Saturdays, Sundays 7am–7pm). You will identify discharge needs and work with the care team to develop safe transition plans that minimize readmissions.
The role requires an RN with 3+ years clinical experience, CCM preferred, and proficient use of Microsoft 365 with strong communication abilities in a fast-paced environment.
Full Time Days Friday, Saturday and Sunday-7am-7pm
The Case Manager (RN) (Prem Wknd) provides case management services to patients and families. This role identifies patient discharge and transition of care needs and coordinates discharge planning activities. The case manager works collaboratively with the patient and interdisciplinary team to develop a safe discharge plan that minimizes the risk of hospital re-admissions.
Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom.
Valid multi-state or State of Oklahoma Registered Nurse License. Certified Case Manager (CCM), preferred.
Minimum 3 years of clinical experience as a registered nurse. Case management or discharge planning experience, preferred.
Independent judgement in making decisions involving non-routine problems under general supervision.
