Job summary

Mount Carmel Dublin is seeking an RN Case Manager to cover weekend part-time shifts. This role directs and coordinates patient care from admission through transfer or discharge, emphasizing cost‑effective resource use and patient advocacy.

You will collaborate with internal and external disciplines to ensure seamless transitions, provide complete clinical handoffs to outside agencies, and identify barriers to efficient stay management while supporting professional development and licensure

Qualifications

• Associates degree from an accredited RN program.
• Current license to practice as registered nurse in the State of Ohio.
• Minimum of 3-5 years recent clinical experience in an acute care setting.

Responsibilities

• Identifies discharge needs, determines level of care, coordinates planning and continuity.
• Ensures all clinical information is provided to outside agencies and communicates with care team effectively.
• Maintains licensure, pursues self-education, and advocates for patient rights while balancing resources and hospital stewardship.
