The RN Case Manager Weekend at UofL Health’s Mary and Elizabeth - UMC campus coordinates patient care across the continuum, performing initial assessments and collaborating with physicians, nurses, and support staff to ensure appropriate levels of care.

You will lead discharge planning, document in the EMR, and advocate for patient and family needs while upholding HIPAA and organizational policies.

Responsibilities

• Performs initial admission assessments and coordinates care with the interdisciplinary team.
• Develops and updates patient care plans and discharge planning in the EMR.
• Coordinates discharge needs with internal and external providers while protecting patient information.
• Documents care and outcomes per Quality Review Audit processes.
• Educates staff and physicians about payor issues, documentation, and compliance.
• Acts as a liaison to optimize resource utilization and patient outcomes.
