Job summary

UofL Health is seeking an RN Case Manager to promote optimal health care outcomes and coordinate patient care across the interdisciplinary team at Mary and Elizabeth - UMC. The role includes conducting admission assessments, validating patient demographics, and ensuring appropriate levels of care and discharge planning.

You will document in the EMR and collaborate with physicians, nurses, and social workers to address barriers to care.

Qualifications

• AS in Nursing or higher nursing credential; Bachelor’s degree required within 3 years of hire.
• Bachelor’s degree in Nursing required within 3 years of date of hire.

Responsibilities

• Promotes optimal health care outcomes and coordinates care with interdisciplinary team.
• Conducts initial admission assessments and ensures appropriate level of care.
• Coordinates discharge needs and ensures documentation in EMR is accurate and timely.
• Advocates for patient/family needs and serves as liaison to medical staff.
• Educates healthcare team about the role of the RN: Case Manager and payor information.
