Job summary

UofL Health is seeking a Weekend RN Case Manager to promote optimal patient outcomes. You will perform initial admission assessments, coordinate care with the interdisciplinary team, and manage discharge planning while ensuring compliance with policies and HIPAA.

The role requires an RN license in Kentucky (or compact), BLS certification, and 2 years of RN experience, with a BSN preferred within 3 years of hire. Excellent communication and organizational skills are essential.

Qualifications

• Associates Degree in Nursing; BSN preferred within 3 years of hire date.
• Two years of RN experience required.
• Current KY RN licensure or compact license to practice in Kentucky.
• Basic Life Support (BLS) certification required.
• Case Management Certification (ACM/CCM) preferred.

Responsibilities

• Promotes optimal management of clinical resources by conducting timely admission assessments.
• Coordinates discharge needs with internal and external providers; develops patient-specific plans of care.
• Educates physicians, patients, and staff on payors, documentation, and compliance issues.
• Acts as liaison for the care coordination team to optimize patient outcomes and resource use.
