Job summary

Divine Rehabilitation and Nursing Center at Shane Hill is seeking a Registered Nurse for full-time or part-time Weekend Warrior shifts (7p–7a) in Rockford, OH. The role emphasizes medication administration, resident care, and coordination with the healthcare team to provide compassionate, high-quality nursing services.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Bachelor’s Degree preferred.
• 1 year of experience as a RN preferred.
• Ability to read/write/comprehend English.
• CPR certification required.

Responsibilities

• Ensure accurate and timely administration and distribution of all medications to residents.
• Administer and document direct resident care, medications and treatments per physicians orders and document all care.
• Obtain and relay information during shift reports to ensure continuity of care.
• Assess and document comprehensive assessments of residents as assigned.
• Develop and implement accurate care plans based on residents’ needs and assessments.
• Provide supervision to Nursing Assistants providing direct resident care.
• Participate in staff evaluations and communicate performance observations to leadership.
• Report changes in residents’ conditions to physicians and supervisors and take follow-up actions.
